We encourage all of our patients with a smartphone or computer with internet access to get a Patient Portal Account. This allows 24/7 access to medical records. You can pay your bills, request refills, get lab results faster, request appointments, seek non-emergent medical advice and request referrals without the hassle of a phone call or waiting on hold. You must have an email account.

It is very important to ensure that you have adequate supply of your long-term medications for conditions such as:

Diabetes, High Blood pressure, Heart Disease, Asthma, Seizures, and Blood Thinners.

  1. Please contact the office 1 week before completion of your final refill. Press option 1.

  2. Call us before the weekend or holidays when office might be closed.

  3. You can also have your pharmacy contact our office.

  4. Use the patient portal for faster response. Avoid telephone calls and phone prompts.

  5. Set reminders on your phones to let you know when refills are due.

DO YOU TAKE YOUR MEDS AS PRESCRIBED?

It is not alwasy easy to keep up with the taking medications on a daily basis, particularly if you are taking many. You are prescribed medications in order to better control medical conditions to prevent worsening or seconday complications. There is clear evidence that taking your medications for high blood pressure, diabetes, high cholesterol significantly reduces the chance of getting complications such as kidney failure, heart attacks and strokes. Of course, there are also potential side effects of medications taken. That is why it is important to have regular followup evaluations by your care team providers and to get recommended evaluations such as labs or other diagnostic studies to evaluate for desired control of conditions being treated as well as to check for complications of those conditions and of medications taken.

We have SAME DAY appointments, Urgent Care appointments. As your medical home, we know you. Instead of going to ER for non-emergency medical issues or going to specialist, Call Us First to evaluate you situation. You can call or use the Patient Portal. Patient Portal is not be used on nonbusiness hours for urgent medical issues.
